Carboplatin Dose (mg) = AUC x (GFR + 25)
(GFR estimated using Cockcroft-Gault Equation, maximum GFR of 125)

Cockcroft-Gault Equation:
CrCl (in mL/min) = sex x ((140 – age) x (weight in kg)/ 72 x serum creatinine (mg/dL))
Sex: Male = 1 and female = 0.85

Consider using an adjusted body weight if overweight or obese (BMI ≥ 25 kg/m2).
Adjusted body weight (kg) = ideal body weight (IBW) + 0.4 x (total body weight [TBW] – IBW)

Consider using a minimum Cr of 0.7 mg/dL to avoid overestimation of CrCl if very low creatinine or in elderly or cachectic patients.
